• FMLA & ST/LT Disability Forms

    All requests for copies of your records from The Right Mind can be provided if needed. You will need to complete a release of information and specify where the records should be sent to. Anything beyond these records are provided as follows:

    Family Medical Leave Act (FMLA) – No forms will be completed until care has been established, generally at least three visits depending on your circumstances.  There is a fee associated with any additional documentation that is completed that is not covered by insurance. There are occasional exceptions to this based on your provider’s assessment.

    Short Term Disability (STD) – No forms will be completed until care has been firmly established, generally 3-6 months. There is no guarantee that these forms will be completed for you, and it is up to the provider if they deem it appropriate.  There is a fee associated with any additional documentation that is completed that is not covered by insurance and you may be asked to schedule an appointment to complete the forms with your provider.

    Long Term Disability (LTD) – No forms will be provided for anyone that has not been under our care for at least 6 months.  These forms are rarely appropriate for someone undergoing outpatient care with a private practice. Generally we will require that you attend a program such as partial hospitalization or intensive outpatient hospitalization. There is a fee associated with any additional documentation that is completed that is not covered by insurance and you may be asked to schedule an appointment to complete the forms with your provider. 

    Social Security Disability Insurance (SSDI) – We do not provide these forms. If you are seeking SSDI you may contact Social Security directly, and they can provide you with an evaluation by one of their professionals trained in assessing this need. As stated above, if you are applying for this type of assistance we will provide your records to the SSDI office directly, but no additional forms will be completed by the provider.
